Weird bump on fairy wrasse

I noticed this evening that my clown fairy wrasse has this strange mark on his head. It wasn't there this morning. It looks like a pimple. He's currently in a 10g qt with a Paracheilinus cf McCoskeri. There's nothing that I can see on it, it's only on the fairy.

It's too big to be ich. We've seen these before and they usually go away on their own or with antibiotics. Are you treating with anything yet?

Yeah I don't think it's ich at all. I'm not treating with anything although I do plan to do prazi and/or metro since he's looking pretty skinny.
Edit: just putting him through ttm right now

Ok. I would say to continue with your treatment and watch the bump carefully. If it gets larger, or turns red then you'll want to move into antibiotics and quickly. You'll take a picture, dose the meds (kanaplex or furan 2), then post about it. Don't wait for a response if it turns red or gets bigger. Treat first, then ask.

Okay it's not larger or red bit there are more of them. Another on its forehead and 5 more evenly spaced around the top part of one eye. Still don't believe that it's ich. Think it might be some other kind of parasite. Heading to lfs (I know I should already have something on hand but was going to pick it up today.)

This is the best I could get, the ones on his eye are on his other side but they are smaller, I can't get a good picture. At the lfs now, picking up kana plex, metroplex, focus,... Anything else?

To me it doesn't look like the lympho in the pictures.

Only the spots in the circle are on the fish. The rest are just artifacts/dirt on the glass.

Could be small infections popping up do to a parasite that we arn't seeing at the moment... like ick or flukes.

You can add kanaplex to the water and still do TTM. Make sure you have plenty of gas exchange while doing this. Also be sure to treat for a minimum of 10 days once you start even if he starts looking better before then.

On the kanaplex directions it says to repeat dosing every 2 days or up to a max of 3 doses. So not 10 days?
