Yellow Tang turning red

Last night I found Yellow Tang (3 inch and was qt,d before intro to DT 9 months ago) with the skin coming off his face. Now the area is red. He was bobbing
around and seemed disoriented. Right side pectoral fin is rotting. Both eyes seem to be slightly popped. I removed him to a hospital tank and added Kanaplex. I have added a picture(poor one). He was eating yesterday but his poop looked like he may have also ingested sand.

125 gallon mixed reef. Dosing vinegar and kalk for top off with RO water
Phos and nitrate are good and I have been dosing Nitrate
Temp 80.2
Salinity 1.024
Ph 8.4
Ammonia 0
Dkh 9

I have recently changed lighting from LED to T5. All other fish seem fine.
2 Maroon clowns and gbta
purple fire fish
sailfin tang
coral beauty ...

and I suspect this bully may have nipped his fins a bit, very minor.
Am I giving the right treatment?
Any help appreciated, I need to save this fish.

If I had to guess, I would say he probably got jammed up in your rock work and cut his face all up. Kanaplex is a good antibiotic to use in this situation. I recently used Bio-Bandage, a topical neonmycin treatment, on a friend's fish that was cut up pretty bad. It worked really well. But I would be afraid of getting it in the fish's eyes in your case.

Thanks Humblefish. I was hoping for you to reply. I did not even consider it but I just rearranged a section of rock in my tank. I just assumed all the fish would
cower on the other side of the tank. With the exception of my female maroon clown that took advantage of my exposed arm. I will update in a couple of days.
By the way I had Kanaplex on hand after reading your recommended cures thread.

Lost the Tang last night. Less than 24 hours after I discovered he was sick. Bummer...
