I have 2 healthy heads on my torch and they both appear to be splitting. One head clearly has two mouths and doesn't look as far into the process of splitting. But the other head looks to just have the one mouth that has a "gaping and stretching" look to it. This head is further along and possibly pinching into 3 heads. I guess my question is more focused on the mouth and when, how, why it's formed since these are clearly different.

Are these the "wall" morphology or the branching variety? I generally try to avoid the wall varieties because they seem to be more susceptible to infections, but these can be beautiful with those large heads. My branching torch will always bud new heads in the flesh near the skeleton. I have never seen one split a head like this in my tank, but apparently it is fairly common...

My torches are all branching and I have only got more heads from splitting not budding. My other euphyllia (hammers and frogspawn) have sprouted new heads.

In that article it does mention new mouth forming after the calcium connects to form the new head which is what one of mine appears to be doing (gaping stretching moth,). But some of my other torches and the other head on this particular piece have multiple mouths before the pinching and splitting.
