Fish is very distant in the pic. Use same light and take closer if possible. Likely an injury but want to confirm

While we are waiting for closer pics and a clear video - when fish show up with ripped fin and a bubble in one eye, that is always from fighting. You need to figure out who is doing the fighting and separate them into two tanks, or with a tank divider. Avoid using an acclimation chamber, those are too small.

The flame angel is pretty beaten up. The goldflake's eye may be from physical trauma, but it could also possibly be from gas supersaturation. Here is an article I wrote on fish eye health:

Just to rule out supersaturation - in home aquariums, that always happens if the system develops an air leak in the suction side of a pump. It could be a pinhole leak in a pipe, but allowing a sump to evaporate down and have it "catch air" is a common cause. If you can rule out any chance of an air leak, then I think you are back to tankmate aggression. However, it could be primary or secondary aggression: In primary, the damage is severe enough to cause the not eating symptoms, etc. In secondary aggression, there is some underlying problem that allows a fish to become weakened and then get attacked. In those cases, the underlying problem needs to be identified and resolved. In this case, it could be flukes. However, you would see some fish scratching/flashing if it were flukes.
