Are the spots raised or appear to be like granules of salt stuck to his side?

This is true. Hippo tangs are very susceptible to HLLE. It is a condition and not a disease. He also had some ich.
No one knows what causes HLLE (Hole in the head disease) but it comes from being in a tank as fish in the sea don't get it. It also can't be cured as even if you stop it, the scarring for the most part will stay.

I have my own theory as to how it happens but the current scientific theory (which I don't agree with) is from the use of carbon.

The ich is just parasites that happened because the fish is stressed from the condition and probably not eating the foods he is supposed to be eating so his immunity faded away.

IMO, that fish can not be cured of the HLLE but that won't kill him for many years.

Good luck, I hope he gets better, I would get some good foods into him like clams or live worms.
