Help with Foxface - disease or physical damage?

My foxface has a whitish spot on the rear of his dorsal fin. I look at my fish daily and noticed it last night. To me it looks like physical damage, but I am FAR from being an expert. He is eating fine, but is swimming back and forth around the side of my tank now. Could this be disease? I could use some help with people more knowledgeable than I. I don’t see any other spots on him nor my other fish. I do have an anemone. I quarantine all of my fish. The last fish to go in my tank was an anthias about 3-4 weeks ago.

Edit: I did clean my tank yesterday and he hates that.

Thoughts on diagnosis and what the best next steps should be?

Here are a few with an orange filter.

I do three 4-day cycles. Quarantine tank is treated with copper power and Seachem kanaplex. Every four days the tank is completely cleaned and wiped down and reset. I do a fresh water dip either in the beginning or between the first and second cycle.

For the anthias on first cycle I slowly raised the copper power. Also the fresh water dip was done between first and second cycle.

How long has the fish been in the tank and what other fish are in there with the foxface?

Foxface has been in the tank around 3-4 months.
2 clowns, six line wrasse, (foxface), lawnmower blenny, 3 chromis, anthias. I haven’t SEEN any aggression between any of the fish. They all seem to get along, but who knows what goes when I am gone.

Is this the spot you are concerned about?

If so, that may be an injury, but it doesn't look like a disease. If it's an injury, then inspect and monitor closely for any changes that might suggest an infection is present. Should an infection arise, you will need to treat with an anitibiotic.

It looks like an injury to me as well - and it also looks like it may be infection. If it worsens I would recommend an antibiotic. The usual would be kanaplex or neoplex - however they may not be as suggessful since the fish has already been exposed to kanaplex. I would recommend that you not use kanaplex for routine quarantine - but only to treat a known illness. Second, your QT does not treat flukes and Third, your copper treatment seems too short (if I'm understanding your program correctly).

the blemish appears as a skin irritation often overcome with good water quality and diet. If other fish are looking good, I would focus on water quality however as a safeguard, you can add Ruby rally pro which is reef safe to keep the fish n check
