For me, I lost a diamond goby once, as that's typical for them.

The only other one was a particular lawnmower blenny. When it came in the mail I thought, "that's the biggest lawnmower blenny I've ever seen." It was caught too late in life, and could not be caged. It was a psycho. LMB #2 after it did just fine for several years, eventually dying of old age.

I lost more fish that way than Star Kist Tuna. :rolleyes:
