Flatworm help, dying hammer

Hello,

Could someone help me with an ID on this flatworm? Maybe 1/4” in size.

I have a fairly new mostly lps tank, I have recent lost of hammer which was doing really well for the first few month. I woke up one morning to a head with exposed skeleton, I took it out to dip the coral again (I dip everything in RS DipX) and the polyp completely fell out. Looks like another head might be going that way too. The coral did look retracted with open mouths for a few days prior.

The only other thing I can thing of is that my wave maker came off of the magnetic mount and fell through the tank when I was away at work. Could have hit the coral and caused some damage?

Parameters and good an stable, as the have been for months. Most recent test today (7th July).

Temp 25-25.5c
No3 10ppm
SG 1.024

Could the flatworm have caused this? Not completely sure if it’s an LPS muncher or not.

That might be a ghost flatworm, but I'm not sure.

Perhaps a pic of the hammer coral and the tank will help generate some suggestions.

Thanks, hammer is looking slightly better today, but not as good as it used to look. I have temporarily moved it to a lower flow area to try and give it a chance to recover… hopefully!
