Video of fish under white lighting will be helpful with assessment.
What is age of tank and is this a new purchase?
What is ammonia and nitrate level as well as water temperature?

The tank is almost a year old and he has been in the tank for about 9 months very happily

Welcome to Reef2Reef!

Determining cause of death after the fact is very difficult. Was the fish thin? My primary concern is that if it is a contagious disease issue, your other fish could be at risk.

I'm sorry to hear that you fish passed. But it would still be helpful if you could answer the above questions.

This will allow us to figure out what happened and possibly, help you to avoid the same mistakes moving forward.

Did you quarantine your fish before placing it in your display?

Hi Jay,

If it were a disease how could it have got into the tank?

I introduced a baby blue tang and two cleaner shrimps about 1 hour before however I doubt that would have caused anything?

No, a new fish less than a day before the loss would not be the cause of any disease, not that fast. Nothing was added in the past 7 to 30 days?

I'm really leaning toward some kind of contaminant.

Were any aerosols used in the house?
Did an exterminator come treat the home?

It was a quick death as well like 2 hours from normal to dead

From your other post, you had this image. It does look like the fish was a bit thin:

Yes I did think he was a tad on the thin side but he's been like that for 9 months and has steadily put on weight

I doubt contaminant or atmospheric issues as it would result in tank wipeout or fish now in distress.

Was the mouth bruised by chance? These guys spook easily and it may have crashed into glass or object
