Just got her in the mail by ups and 30 min i acclimated her and she was doing the same thing In the bad took her out mas she swam around a little then just went upside down ammonia 0 nitrite 0 salitnty 1.020

tank size and age? that acclimation was pretty sudden for a salinity change like that. is your tank at a salinity 28.8 ppm? pretty sure it should be around 35

Although swim bladder disease is possible, I wouldn't rule out other illnesses. Oftentimes fish experience stress from transport and acclimation, and a fish as sensitive and difficult as a spiny boxfish can react very poorly. You mentioned in the title that it looks like the fish has ich. Can you get clearer pictures of the tormonts? If there is an active disease, you may want to consider calling where you ordered it and explaining the situation, because shipping out a sick fish is unacceptable. It is most likely struggling because it just suffered a traumatic experience, and will either get used to its new environment, with your help, or die.
