Rhomboid infection/wound

I noticed today my rhomboid has a white spot/wound near his tail. The scales in the area are raised and the area is noticeably puffy. I don’t really have any rocks that could fall in him and my tank is pretty passive but I do have 2 anthias and 2 tangs that maybe could have attacked him.

He’s swimming fine and eating Well. I watched him for a good hour today and I didn’t see any chasing. Could it be anything else? Should I start up a QT And medicate or observe for a day or two? I haven’t added anything to the tank in quite some time so I don’t think it’s a parasite. Here’s a quick vid, sorry for the jitters he’s fast even with the wound

Looks like a wound or injury.
Treating with methylene blue is an old school method that still works well.

And then with antibiotics if necessary. Look for white filmy areas, redness, or necrosis.
