Some questions about torch coral recovery

One month ago, i changed my landscape in my tank.

After that, my torch coral become poor extend on & off, and also noted the surrounding tissue atrophy. :(

My parameter: NO3: 1-2.5, PO4: 0.01-0.05, KH keep as 8.3,

How can I make a full recovery to my torch? Especially the surrounding tissue...

I felt so sad about this condition. :(

I've revived both Duncan's and hammers using an iodine dip. I dipped them for 5-10 minutes and returned them to the tank in a low flow area and they started to recover in a few days. Do you have pics of the torch?
