Have done some searching on ion uptake via fish gills. It seems like it could be possible that latnhanum can be taken up through gills. If that is the case, lanthanum could theoretically mess with phosphate homeostasis in the blood of the fish which could cause toxicity.
Anyway, never add more lanthanum than you have phosphate. Following the directions of the calculator that I linked to earlier could be a good idea. Never add more lanthanum than required to reduce phosphate by more than half and then repeat with lower and lower dose.
 

Freshwater filter only or is it? Have you ever used an HOB filter on a saltwater tank?
