Looks like a rash from a scratch injury and should heal on its own however it seems butterflies take longer to heal and I would at Minimum add Ruby Rally Pro but ultimately, if it becomes more sore treat the fish in a separate tank using Seachem neoplex with added aeration and focus on ammonia levels with a reliable test kit.

Visually, I can't make a diagnosis on just a photo, more background history is needed:

How long have you had the fish?
How long has it had this lesion?
Is it showing any other symptoms? (rapid breathing, not eating, etc.)
What are the fish's tankmates?

Hi Jay
Only had the fish couple of weeks but when I put him in my tank my emperor angel did keep attacking him so I moved the emperor that day to isolation tank.

Other tank mates are silver belly wrasse a couple of clowns couple damsels and orange spot file fish.
The fish seems to be eating fine and doesn’t seem to be breathing heavily

Thanks
