GREEN BUBBLE TIP ANEMONE FELL OFF/MOUTH GAPING/ PLEASE HELP

Hi there guys, had my green BTA for a month and a half now perfectly fine. Suddenly today while it was doing its daily deflatin, you know getting the old water and poop out, he suddenly fell. I put him on a rock, but then he turned oval shaped, mouth gaping but very narrow and very strechted. Then he moved down to sand and fell upside down. I flipped him over but then he turned into a sandwhich shape. Then upside down, sort of.

If the tentacles are always that stubby the Anemone was likely declining due to lack of sufficient lighting, not receiving enough nutrition from zooxanthellae.
How old is the system?
What lighting type and brand?
Double check the water chemistry.

How old is the system?
What lighting type and brand?
Double check the water chemistry.
System is almost 1 year old.
I kinda forgot what light I was using but the brand is Spectra.
The water chemistry is perfect except for nitrates which is 50, been like that for 2-3 weeks.
At first I placed the anemone at a 200 par place but then it moved on its own to a 50+- par place. I didn't want to move it because it moved there on its own and I didn't want to disturb it, I coudn't move because hook inside rock crevice,.

My RBTA split first day in the tank. The small one has looked like that for months, but every now and then still shows some sort of sign of wanting to come back.

I am no expert, but if that guy re-attaches somewhere, I'd just try and wait it out. If it stays there and continues to decline I'd probably remove it :(
