HELP. Clownfish appears to have something wrong under its head.

My dark clown fish recently developed this spot under it's head. I have never dealt with fish disease before as I keep very few fish.
Until now the fish seems to be eating and acting normal.
Tank parameters are all within good range and have been stable for some time. I have had these clowns in this tank for about 3 years and have never seen this until a few days ago.
My tank it pretty full or corals although the picture does not show it.
Only other fish in the tank is a Benghai Cardinal. Otherwise there are some small hermits and snails.

Please help me ID this.

The pics aren’t really clear enough to say for sure, but the two causes for a lesion right there would be either a bump injury or a goiter tumor.

This flesh that is raised seems to be loose. It's almost like it is shedding the skin in that area.
If it is an injury, is there anything I should do?
