Is this a bacterial infection? Currently, this fish has been in quarantine for 40 days with four other fish. The others are very healthy and show no signs of disease. IMG_8894.png IMG_8893.png

This is a skin irritation which has produced mucus cones likely mixed in with ich. I realize you are in Asia. Do you know the water quality in your area/region?
As for treatment, once again, you will want to use coppersafe for a full 30 days not to exceed 2.25 and how have you been testing previous copper treatments?

Yes am from VietNam . I suspect it has a Bacterial Tufts & Viral Nodules . “It’s not ich because there are four fish in the tank, and after two weeks, there are no signs of it spreading to the others. Even sensitive fish like the hippo tang haven’t been affected.”

Again - this is mucus plugs likely mixed with ich and often other fish will not show any signs but keep an eye on them

It sure looks like ich mixed with mucus plugs.

Is the fish currently in copper? At what dose?

Do you have a microscope? In some cases, the only way to get a good diagnosis is through a skin scrape.

Currently, the fish is not being treated with copper because I don’t see any signs of the disease spreading to other fish in the tank. Interestingly, the tiny spots only appear in the early morning when I turn on the lights or at night when I shine a flashlight on the fish. I am considering using copper or giving it a formalin bath individually . Any advices for me ?

Formalin dips are not effective against ich, and there is some bad advice out there telling people to dip fish at 250 ppm. Never go higher than 167 ppm for tropical fish.

I would treat the tank with copper power at 2.25 ppm
