What kind of worm is this?

As attached. May i know what kind of worm is that?
Should i remove it?
It is found at night after light off. Saw it swimming frantically before the video

With it swimming, it may be an epitoke (a bristleworm in its reproductive stage), but there are a few kinds of bristleworms that can swim at any stage.

How many tentacles does it have on its head?

Thank you for your reply. From the video i took I can't seem to count the tentacles. But maybe 2?
Plus I've not been able to find him again after that day.
The way the worm swim is creeping me out. Haha

Haha, understandable - if you find it again and can trap it in a container of tank water (preferably a black container), you can get some more pics I can try to offer a better ID; I don't recognize the worm from the video though.

That said, most polychaete worms we get in our tanks are harmless/beneficial, so I wouldn't be too worried unless things are mysteriously dying.
