They're turbellarian worms often called black ich and they primarily feed off the given fish itself and can be successfully treated in quarantine with PraziPro (praziquantel) and even given a freshwater dip in water the same temperature as display tank in a clean bucket prior to PraziPro.

Increase oxygen with an air stone during treatment as Prazi often reduces oxygen slightly.
Treament is 8 days followed by water change and one more 8 day interval if needed- Dose for ONE eight day interval, not daily

I’ve owned it for over 2 years now and it was a good size when I got it, I assume it was back from when they where still collected from HawaiiFormalin tends to work better than praziquantel on this issue, because the worms are pretty deeply seated, and are not true flukes. Formalin is tough to source in Europe and the UK now. On the other hand, this issue is often self-limiting and goes away on its own.
My question is; this looks like a rather large tang. Back spot disease (turbellarians) are usually only seen in newly collected tangs. Do you know where it was collected? There are some being collected in Ogasawara Island, but they are really costly in the US.
