Hammer problems

Has anyone ever had a hammer do this? One head closed up and produced a white cap to close it inside the skeleton. It did not bail out yet. I’ve water changed 25% and changed flow to lower and none also moved it out of the light and nothing helped it was mad 2 days then I woke up to this any help is appreciated!

In 25 years, I've never seen a hammer do that.

I knew it was a unique situation. I peeled it back and black liquid was inside . Lost half the head and the other head is still irritated but nothing els was bothered. I wonder if a remitted snail got in the head some how cause it had that like dead algae lookin, grey stringy fuzz , around the head I kept cleaning off and thought the flow was the problem. It looked like the net remitted snails throw out. Idk.
