How long have you had the puffer, and is he still eating and swimming normal? Was he puffed up at some point and got sucked in to a power head?

That has to be an infection of some sort. Are you able to put him in a QT and medicate?

Well I'm thinking infection. The meds would be- in this case I think we should hit it with a wide spectrum- Kanaplex, furan 2 and metroplex. Follow the dosing directions on each individual med and keep the treatment going for 10 days minimum. Boy oh boy, this is a weird one.

Is there any way he could have been stung by a coral or anemone? Picked on by another fish?

I haven't either really, but I've done a lot of this and this is my best bet. I'll put a call out for @Humblefish because A. I think he'll find this interesting and B. He's more experienced than I am. Hopefully he'll have time to get online during his vaca

Ok. So I'd get him into QT ASAP. That's moving really fast! If it were me, I'd do a FW dip on the way to the QT then start meds right now. If it weren't moving so darn fast, I'd say you have time to wait until tomorrow, but at this rate he'll be fully purple with this stuff by morning.
