Dwarf Lionfish Bloated

Hello, I am new to this forum and was hoping to get some advice/ insite. We have had this dwarf lionfish for about 3 weeks. We typically feed him ghost shrimp and the occasional guppy every 3 to 4 days. Yesterday into today he has appeared bloated, hanging out towards the top of the tank. His color is also lighter than before. The tank has been setup for about 4 months at this point.

The other corals and fish seem happy and healthy.

Ugh - see the "pineapple" look to the scales? That is due to fluid in the body causing the scales to spread apart. That is due to edema or ascites. In freshwater fish, that is called "dropsy". The root cause is kidney or liver failure. As with people, there is no good treatment for that (which is why transplants are needed).
 

Freshwater filter only or is it? Have you ever used an HOB filter on a saltwater tank?
