It looks like a skin irritation or infection. Is his breathing normal?No, nothing added at all
The only thing that could have done it would be my carpet anemone but they have been in the tank together for the past 2 months

Yes I was asking if you qaurantine before adding to your main tank. If you haven't added anything recently then in theory it shouldn't be velvet or ich but nothing is 100%. Just trying to help I know it sucks to have these kinds of things happenWhen sick ? Or new ?

Darn dude sorry to hear that. It sucks when you get attached to a fish and think it was your fault he died.Well what ever it was didn't take long - he passed tonight after moving over to Q Tank
